Registering with the practice
To register with our practice, we require all of the following:
1. Completed forms - Download here.
GMS1 - Health Promotion Questionnaire - Ethnic Origin - Patient Registration Declaration*
2. Passport or a Birth Certificate (we will photocopy)
3. Evidence of current address (e.g. bill)
The Health Authority are now asking new patients to provide evidence of their full name and date of birth when they register at Sid Valley Practice. We are able to accept a passport or birth certificate for this purpose.
We are unable to register you without these details. We will also need evidence of the address where you were previously living which should include the postcode.
We are open for new patient registrations every weekday 9.00am to 1.00pm and 2.00pm to 4.00pm. Please telephone 01395 512601 & speak to the Enquiries team (option 3).
NEW PATIENT REGISTRATIONS SHOULD BE MADE IN PERSON EITHER BY THE PATIENT OR THE PATIENT'S REPRESENTATIVE.
*The Patient Registration Declaration form only needs to be completed if you have been living outside of the UK in the last 12 months.
