If you have changed address and have not yet informed us, you can do so via the form here.
Remember to inform your hospital of your new address if you are undergoing any treatment or awaiting an appointment.
If your new address is outside our registered area (see map for boundaries) you will need to re-register with a doctor that covers your new address.
