District Nurses
The District Nurse team is based at the Sidmouth Health Centre. Our aims are to assist with a safe discharge from hospital into the community. We support our patients with skilled nursing care within their own homes and/or other community settings and we try to prevent hospital admission wherever possible.
Our team identify patient’s needs and provide skilled research based clinically effective nursing care to patients including support to carers, primarily in the home environment. We integrate with social services, voluntary and private agencies to deliver continuing health care within the community setting.
We encourage patients and assist them to maximise their level of independence and quality of life in the context of their own lifestyle with a view of discharging from our caseload.

Assessment of patients who are housebound.
Long-term management and support of patients with chronic degenerative conditions.
Wound assessment and management of dressings.
Specialist Leg ulcer clinic.
Post operative care/follow up at home.
Educating and supporting patients and carers.
Administration of injections in the home.
Terminal/Palliative Care.
Diagnostic investigations including blood tests, ECGs, bladder scans, doppler tests.
Continence assessment.
